Electrolytes and hydration on semaglutide or tirzepatide

A lot of our daily fluid and salt comes from food. When a GLP-1 cuts appetite, fluid and electrolyte intake can drop quietly alongside it. Add nausea, vomiting, or diarrhea and dehydration becomes a real risk. Prescribing information for these drugs notes kidney injury linked to dehydration from gastrointestinal side effects.

Signs to watch for

Headache, dizziness when standing, fatigue, muscle cramps, dark urine, and constipation can all point to low fluid or electrolytes.

Habits that help

  • Drink on a schedule, not on thirst. Fullness blunts thirst. Small amounts through the day work better than large drinks with meals.
  • Add electrolytes without sugar. A sugar-free electrolyte powder once or twice a day is a common ND suggestion, particularly on days with nausea or exercise.
  • Salt your food normally unless your clinician has told you to limit sodium.
  • Eat potassium foods when you can: avocado, potato, beans, yogurt, bananas.
  • Plan for heat and workouts. Sweat losses add up quickly on reduced intake.

Who needs extra care

People with kidney disease, heart failure, high blood pressure, or who take diuretics or other blood pressure medicines should not change salt or electrolyte intake without their clinician’s guidance.

When to get help

Inability to keep fluids down, very little urine, confusion, or fainting needs urgent medical care.

Safety information

GLP-1 medications are prescription drugs with risks including nausea, vomiting, pancreatitis, gallbladder disease, kidney problems from dehydration, and a boxed warning for thyroid C-cell tumors in rodents. Compounded versions are not FDA-approved.
